Requirements: Owner Operators Qualifications: Applicant, If you have been diagnosed with any of the following conditions please take a moment to review what the DOT physician will need from you in order to pass you on your upcoming examination. HIGH BLOOD PRESSURE - If you are currently taking medications to control your high blood pressure be sure you either know the name and dosages of each pill or bring a list. Your blood pressure must be controlled at the time of the exam, which means you must have a reading below 140/90. If you are running higher than 140/90, you will be given 3 month temporary certification that will allow you time to seek better control of your hypertension with your primary care physician. At the time of your recheck with the DOT physician, you will need to bring a note from your treating physician explaining that the conditions has been addressed. (A copy of your doctor's office notes would be sufficient). Blood pressure on a recheck with the DOT doctor must be less than 140/90. If you feel your blood pressure reading on the day of the DOT exam is elevated due to loss of sleep, stress, nervousness, etc. you may choose to return the next day for another blood pressure check. If on recheck your blood pressure remains elevated the steps outlined above will apply. If you experience the "White Coat" hypertension phenomenon, best described as an elevation in blood pressure resulting from the anxiety, apprehension, stress, of being in a doctor's office, it will be necessary for you to have several recorded blood pressure reading from a home monitor as well as documentation from your physician showing a normal blood pressure reading in his/her office. HEART ATTACK - If you have a medical history of a heart attack you will need to provide to the DOT physician a copy of your hospitalization records. Those medical records will identify the type of heart attack, seriousness of the attack and method of treatment. In addition, you will need to furnish medical clearance from your cardiologist. The DOT requires that you are rechecked annually and that every two years you have a repeat treadmill test by a cardiologist. ANGINA - Angina is a constricting chest paid. For purposes of qualifying under the DOT rules the same conditions apply as for heart attack. Be prepared for your exam by providing clearance from the treating cardiologist, medical records and documentation showing you have passed a treadmill test within the past 2 years. SLEEP APNEA - Applicants with this conditions will need to show compliance with treatment by providing the DOT physician with medical notes from the treating physician. DOT recommends that the sleep latency test be administered every year. Applicants who comply with the recommended treatment and comply with our DOT physicians's requirements will be issued a one-year card. DISBETES - A diagnosis of Type I diabetes is DOT disqualifying. Type II should be adequately treated. For purposes of passing your DOT exam you will need to provide medical documentation showing regular follow-up with your primary care physician, evidence that you hemoglobin A1C is being checked and your current A1C level. The DOT physician is looking for evidence that the diabetes is treated well and for documentation that there have been no episodes of low blood sugar due to the medications. DEPRESSION - Applicants who have a new diagnosis of depression are DOT disqualified. If you have been diagnosed for some time and are well controlled you can anticipate at your DOT exam that the physician will take a history from you concerning your diagnosis. If you have ever been hospitalized for your depression you will want to provide discharge notes from the treating physician. If you take medication to treat your depression you will need to provide names of medications and dosage. However, if you require anti-anxiety medications such as valium or lorazepam you will not be qualified to drive under the DOT regulations. The physician will deal with individuals treated for depression on a case-by-case basis. SEIZURES - Will almost always be disqualifying unless the applicant has a very remote history of seizure and has been seizure free and off medications for ten years. Medical records will be required. |
